چکیده

Purpose: To study the safety and efficacy of intranasal desmopressin and intramuscular tramadol versus pethidine for treatment of renal colic. Materials and Methods: A total of 90 adult patients who presented with renal colic to the emergency wards were recruited in this study. The patients were randomly assigned to receive 100 mg intramuscular tramadol, 40 μg intranasal desmopressin, or 40 μg intranasal desmopressin plus 100 mg intramuscular tramadol. The severity of the pain was assessed using Visual Analogue Scale. Results: The studied patients consisted of 49 men and 41 women with the mean age of 35.20 ± 13.26 years (range, 16 to 82 years). There was no statistically significant difference regarding the mean age (F [2, 89] = 2.98, P = .056) and gender differences (x2 = 3.3, df = 2, P = .19) in three groups. There was also no statistically significant difference considering pain relief in 3 studied groups (P = .2). Conclusion: We concluded that narcotics such as pethidine can not be replaced by tramadol in patients with renal colic, but tramadol, desmopressin, or both in combination can reduce pethidine requirement.

Redefining the Stone Belt: Precipitation Is Associated with Increased Risk of Urinary Stone Disease.

OBJECTIVES The American Southeast has been labeled the "Stone Belt" due to its relatively high burden of urinary stone disease, presumed to be related to its higher temperatures. However, other regions with high temperatures (e.g., the Southwest) do not have the same disease prevalence as the southeast. We seek to explore the association of stone disease to other climate-associated factors beyo...

Novel use of a ureteroscopic stone entrapment device to prevent antegrade stone migration during percutaneous nephrolithotomy.

The Accordion stone management device is a hydrophilic microcatheter-based tool that blocks stone migration. This instrument has been described for use during ureteroscopy to prevent retropulsion of stone fragments. We used this device during percutaneous nephrolithotomy (PCNL) in 30 patients to prevent antegrade migration of stone fragments (group 1), and find that the device is easily inserte...

Factors affecting the success of ureteroscopy in management of ureteral stone diseases in children.

PURPOSE We retrospectively investigated the factors that affect the success of rigid ureteroscopy in the pediatric population for the management of pediatric ureteral stones. PATIENTS AND METHODS We present a retrospective review of 61 consecutive pediatric patients who underwent 66 rigid ureteroscopy procedures for ureteral stone management. The effects of sex, age, stone diameter, stone loc...

Outcome of extracorporeal shockwave lithotripsy monotherapy for large renal calculi: effect of stone and collecting system surface areas and cost-effectiveness of treatment.

The treatment options for large renal calculi are controversial. We report on our experience with 65 treatments of renal calculi > 3 cm using extracorporeal shockwave lithotripsy (SWL) monotherapy. We stratified our results according to stone and collecting system surface areas (measured by computer image analyses), stone location, and stone type. The overall success rate of SWL monotherapy was...

Endoscopic Stone Measurement During Ureteroscopy.

INTRODUCTION Currently, stone size cannot be accurately measured while performing flexible ureteroscopy (URS). We developed novel software for ureteroscopic, stone size measurement, and then evaluated its performance. METHODS A novel application capable of measuring stone fragment size, based on the known distance of the basket tip in the ureteroscope's visual field, was designed and calibrat...

Prevention of stone migration with the Accordion during endoscopic ureteral lithotripsy.

BACKGROUND AND PURPOSE Endoscopic lithotripsy is often prolonged secondary to the retrograde migration of calculous fragments. Various balloons, baskets, and other devices have been used to prevent this migration. Our purpose is to analyze the effect of the Accordion(®) on stone migration and overall efficiency during lithotripsy. PATIENTS AND METHODS We prospectively evaluated 21 patients wi...
